Instructions to apply for a certified copy of a Birth Certificate
To apply, you must complete the appropriate application AND provide one of the following Proofs of Identification:
- Valid Driver's License
- Valid DMV Driver's Permit
- Valid Non-Driver's ID
- Valid Passport
- Valid NY State Benefit Card w/ Photo
*If you're submitting an application by mail, we still need this proof!
A photocopy will suffice.*
WHO MAY APPLY FOR A CERTIFIED COPY OF BIRTH CERTIFICATE
- Child listed on Birth Certificate
- Parent listed on Birth Certificate
- Legal Guardian (must provide proof of guardianship)
- Descendent (must have a court order from a NY State Court of competent jurisdiction AND prove lineage)- If you don't need a certified copy, please fill out a Genealogy request instead.
MAIL REQUESTS:
- If applying for a copy of a Birth Certificate by mail, please include a phone number on your application. If a phone number is not included, it may delay your request.

Death Certificate
Instructions to apply for a certified copy of a Death Certificate
To apply, you must complete the appropriate application AND provide one of the following Proofs of Identification:
- Valid Driver's License
- Valid DMV Driver's Permit
- Valid Non-Driver's ID
- Valid Passport
- Valid NY State Benefit Card w/ Photo
*If you're submitting an application by mail, we still need this proof!
A photocopy will suffice.*
WHO MAY APPLY FOR A CERTIFIED COPY OF DEATH CERTIFICATE
(with official proof of relationship)
- Spouse (proof is marriage certificate)
- Siblings (proof is your birth certificate showing you share at least one parent with the deceased)
- Children of the deceased (proof is your birth certificate showing the deceased is your parent)
- Parents of the deceased (proof of your id required)
- Descendent (must also submit a notarized Affidavit to Request Certified Copy of Death Certificate AND documentation demonstrating relationship) - If you don't need a certified copy, please fill out a Genealogy request instead.

Marriage Certificate
Instructions to apply for a certified copy of a Marriage Certificate
To apply, you must complete the appropriate application AND provide one of the following Proofs of Identification:
- Valid Driver's License
- Valid DMV Driver's Permit
- Valid Non-Driver's ID
- Valid Passport
- Valid NY State Benefit Card w/ Photo
*If you're submitting an application by mail, we still need this proof!

WHO MAY APPLY FOR A CERTIFIED COPY OF MARRIAGE CERTIFICATE
- Bride, Groom, or Spouse listed on the original Marriage License
- Descendent (must have a court order from a NY State Court of competent jurisdiction AND submit death certificates for both parties; applicant's identification; proof of relationship; and a completed and notarized Affidavit to Request Certified Copy of Marriage Certificate) - If you don't need a certified copy, please fill out a Genealogy request instead.

The location of your ceremony does not determine that the certificate was originally obtained here. Marriage certificates can be obtained anywhere in New York State.***
