Instructions to apply for a copy of a Birth Certificate. To apply, you must complete the appropriate application AND provide one of the following Proofs of Identification:

• Valid Driver's License
• Valid DMV Driver's Permit
• Valid Non-Driver's ID
• Valid Passport
• Valid NY State Benefit Card w/ Photo

*If you're submitting an application by mail, we still need this proof!

A photocopy will suffice.*

WHO MAY APPLY FOR A COPY OF BIRTH CERTIFICATE 

1. Child listed on Birth Certificate
2. Parent listed on Birth Certificate
3. Legal Guardian (must provide proof of guardianship)

MAIL REQUESTS:

1. If applying for a copy of a Birth Certificate by mail, please include a phone number on your application. If a phone number is not included, it may delay your request.
2. Please mail your request(s) to:

                 Manchester Town Clerk 

            1272 County Road 7  

                     Clifton Springs, NY 14432

***NOTE: Birth certificates on file at the Manchester Town Clerk's Office are ONLY for HOME BIRTHS that took place within the Township of Manchester***

• Valid DMV Driver's Permit
• Valid Non-Driver's ID
• Valid Passport
• Valid NY State Benefit Card w/ Photo

*If you're submitting an application by mail, we still need this proof!

WHO MAY APPLY FOR A COPY OF DEATH CERTIFICATE 

(with official proof of relationship)

1. Spouse
2. Siblings
3. Child of the deceased
4. Parent(s) of the deceased
5. Lawful Representative of the deceased

1. If applying for a copy of a Death Certificate by mail, please include a phone number on your application. If a phone number is not included, it may delay your request.

***NOTE: Death Certificates on file at the Manchester Town Clerk's Office are ONLY for deaths that took place in the Town of Manchester. We do not have death certificates for any villages***

• Valid DMV Driver's Permit
• Valid Non-Driver's ID
• Valid Passport
• Valid NY State Benefit Card w/ Photo

WHO MAY APPLY FOR A COPY OF MARRIAGE CERTIFICATE 

1. Spouses listed on the Marriage License
2. Lawful Representative (with official proof)

1. If applying for a copy of a Marriage Certificate by mail, please include a phone number on your application. If a phone number is not included, it may delay your request. 
2. Please mail your request(s) to:

***NOTE: Marriage Certificates on file at the Manchester Town Clerk's Office are ONLY for marriage license applications that were originally issued by the Town of Manchester***
