ACCESSIBLE PARKING PERMIT APPLICATION FOR SEVERE DISABILITY

If you live in the TOWN of Manchester, please have your doctor complete this form and return it to the Town Clerk's Office where we will issue you a parking permit. If you live in one of the VILLAGES, please go to your village office with this completed form, and they will be able to assist you there.

Please be aware that these tags are only to be used by the person that they were issued to. Tags are not allowed to be given, loaned, or otherwise used by anyone other than the person with the disability. Parking tags have YOUR information on them, and anyone violating these rules can face steep fines. Each parking tag is issued to a person, not a vehicle, so you may use the tag in any vehicle so long as you're the person using the parking tag and the tag was issued to you. This also means that you can use the tag while riding in someone else's vehicle. You may bring the below form to your doctor, or they will often have a copy on file.
